LPN (Licensed Practical Nurse)
Overview
Crystal Springs is offering a $5000 (based on 40 hours per week) sign on bonus for newly hired full time LPNs.
Crystal Springs provides $4.50 per hour retention pay. Retention pay through June 30, 2024 and is contingent upon continued funding.
Crystal Springs is seeking LPNs to come and join our team. We offer flexible schedules, retention pay, and a $5,000 sign-on bonus for full time positions and a $2,000 sign-on bonus for part-time positions. We welcome new graduates. Our LPNs coordinate care and provide direct nursing support to adults with intellectual and developmental disabilities in a safe, empowering, and satisfying work environment.
LPN reports to the Director of Nursing and are responsible for the following:
- Perform bimonthly in-home audits and medication and MAR reviews
- Maintain health records according to DDS standards assuring that necessary documentation is up to date.
- Assess and coordinate healthcare services for individuals and initiate follow-up and communication of all medical concerns to the Director of Nursing, individuals’ team, parents/guardians, Area Director, medical providers, and any other vendor/s involved in the individuals’ care.
- Maintain current medication and treatment orders noting any side effects/adverse effects and report to appropriate people the Director of Nursing, individuals’ team, parents/guardians, Area Director, medical providers, and any other vendor/s involved in the individuals’ care. Assist in training staff in medication policies and procedures.
- Perform physical assessments, including vital signs such as blood pressure, temperature, and weight. Provide basic wound care including cleaning and bandaging injured areas when needed. Administer medications when needed/required in adherence to MAP regulations. Provide G and J tube care as needed.
- Review ISPs for current knowledge of programming in self-medication, behavior plans, hygiene skills, and physical therapy plans.
- Review all appropriate tracking logs (BM, menses, etc.) and provide additional training/retraining when necessary.
- Order medication from pharmacy as needed.
- Provide MAP training/education to staff pursuing MAP certification.
- Meet with Director of Nursing for supervision and direction as needed to ensure appropriate medical health care is provided for individuals
- Attend staff meetings to present all current medical/health issues and concerns to team as well as upcoming medical appointments.
- Attend all designated mandatory in-services.
Qualifications
To implement procedures and services including medical interventions, or additional duties, as assigned by the Director of Nursing:
- LPN with current MA license.
- LPN: New graduates welcome, 1 years of experience as LPN preferred
- Valid Driver’s License

- Full Time - 32 hours per week
- Monday to Friday
Schedule
- Up to $32.20 per hour
- Non-traditional hours: Monday through Friday with one day off during the week
- 10:00 am - 6:00 p.m. no weekends or holidays
- Rotating weekend on call-responsibility.
- Per diem hours also available
